This registration form is for current members of GP partners Adelaide. There is currently no public registration available.
By registering on the GP partners Adelaide website you will gain access to content that is provided exclusively for members which can include (but is not limited to) tailored news and event information, promotions and members forum.
User name Your user name is the name you will require when logging in, it is not public and the automatically created name should be sufficient in most cases. However if you have a preferred username then you may enter it here.
Display name Your display name may be publically visible in the future when we add more features to the site so if you wish to maintain some anonymity then you can enter in a display name otherwise your username will be used automatically.
The information you provide on this form is subject to our Privacy Statement. No information you provide on this form will be publicly visible on the site unless you explicitly allow it (which you can do once your registration has been completed and your account activated).
Step 1 - Fill out the form below. Registering is easy, however you may have to wait a couple of days before your registration is processed. The first step is to fill out the small form below, the required fields have been marked and you must enter these. The username and display name fields are not required and if you do not enter anything into these fields we will automatically regenerate a user name and display name for you by taking the first letter of your first name and appending your last name to it. A short summary of username and display name fields have been provided to the left for those who may be unfamiliar with the terminology. Step 2 - Activation Once you have successfully completed the registration form your account will be created however it will be in an inactive state. A GP partners Adelaide staff member will need to manually confirm that you are a member of the division. This could take minutes, or this could take up to three days, if there is a problem confirming you are a member we will contact you via email. Once we have confirmed you are a member your account will be activated, we will send you a notification of this via Email with information of how to log on.
