GP partners Adelaide is pleased to participate in the new Mixed Wave of the Australian Primary Care Collaboratives (APCC) Program on Diabetes Management and Diabetes Prevention. GP partners has recruited 6 practices and the first learning State workshop was conducted in March 2011.
The program provides a rare opportunity to get together with colleagues at a series of learning workshops to exchange ideas, share experiences and learn about practical quality improvement skills using the successful ‘Model for Improvement’.
Twenty three practices in our Divisional area are currently participating in various APCC programs and have celebrated successes in data cleansing, uniform clinical coding, up-to-date registers and recall and reminders as well as improvements in the practice appointment systems.
Practices can expect to:

Improve the health of patients;

Enhance practice clinical reporting and functionality;

Improve data quality for patients with Diabetes and CHD;

Build a stronger practice team and improve practice efficiency and performance;

Enhance quality improvement skills of practice staff;
- Receive a financial practice incentive to participate;

Receive professional development points for GPs and Practice Nurses.
