Chronic Disease Management and Prevention

Chronic Disease Management (CDM) in General Practice involves appropriate prevention, early identification and best practice management strategies. As general practitioners are usually the first point of contact in the health system you play a key role in the primary intervention, prevention, diagnosis and management of chronic disease in the community.

The Division offers the following to assist you:

Diabetes Management in General Practice 11-12

General practitioners continue to provide most of the medical support to people with type 2 diabetes. This is the most recent edition of this guide and it plays an important role in providing a readable summary of current guidelines and recommendations from various sources on the management of type 2 diabetes in the general practice setting.

Chronic Disease Management Resource Manual

The January 2012 update of the Chronic Disease Management Resource Manual is now available. This resource aims to assist in understanding the systems and incentives for Chronic Disease Management in General Practice. The new manual has been updated to incorporate recent Medicare changes.

To ensure that you have the most up to date version of the manual we encourage you to use it from our website. The website version can be downloaded if you prefer a hard copy however the online version will provide quick access to the links within the manual. It may be useful to add a short cut to the manual on your desktop.


The Chronic Disease Management Resource Manual includes five sections which focus on specific topics. Each of these sections can be downloaded below.

MBS Item Number Flip Chart

This MBS Item Number flip chart is designed to be a quick reference guide to MBS items for Chronic Disease Management and Health Assessments. Updated July 2011, the latest version of the flip chart reflects the current rebates for each item.

Printing

The Flip chart has been designed so that it is easily viewable on a computer however it can also be printed and bound as a flip chart for quick reference. To create your own printed copy of the flip chart simply download the print friendly pdf version here and print it. Once printed, cut or guillotine the pages along the lines and then bind or staple the pages together in order of smallest to largest.

Referral Pathways Charts

These charts have been developed by GP partners Adelaide, in collaboration with Adelaide Northern Division of General Practice and Adelaide North East Division of General Practice, to assist GPs and practice nurses in identifying local programs and services to support patients who are at risk or diagnosed with the disease. They include the relevant MBS item numbers to be considered as well as support resources.

The charts will be regularly updated; to ensure that you are using the most recent versions of the charts we encourage you to access them from our website.

Aboriginal and Torres Strait Islander Health Check Resources

These health checks are Medicare services for Aboriginal and Torres Strait Islander people.The purpose of the health checks are to ensure that Aboriginal and Torres Strait Islander people receive the optimum level of health care by encouraging prevention, early detection,diagnosis and intervention for common and treatable conditions that cause considerable morbidity and early mortality. Better Start for Children with a Disability Initiative
Tuesday, 7 February 2012
MBS Item 139 (for General Practitioners) and MBS Item 137 (for specialist or consultant physicians, on referral from a medical practitioner) are new item numbers that commenced July 2011 as a part of the Better Start for Children with a Disability Initiative.

The items are available to provide assessment, diagnosis and development of a treatment and management plan for a child aged under 13 years with an eligible disability including:
  • sight impairment that results in vision of less than or equal to 6/18 vision or equivalent field loss in the better eye, with correction; 
  • hearing impairment that results in a hearing loss of 40 decibels or greater in the better ear, across four frequencies; or permanent conductive hearing loss and auditory neuropathy; 
  • cerebral palsy; 
  • Down syndrome; or 
  • Fragile X syndrome. 

Through these items a child can access Medicare rebates for 4 assessment and 20 treatment services from eligible allied health professionals including audiologists, occupational therapists, optometrists, orthoptists, physiotherapists, psychologists; and speech pathologists. Children with autism or any other pervasive developmental disorders (PDD) such as Rett’s Disorder, Asperger’s Syndrome or childhood disintegrative disorder may be eligible for the Helping Children with Autism program. However, children with both autism/PDD and an eligible disability can only access one program or the other, not both.
