Coordinated Veterans’ Care (CVC) Program



The Department of Veterans’ Affairs (DVA) has commenced a new Coordinated Veterans’ Care Program (CVC) which is designed to engage GPs and practice nurses in the management of veterans’ chronic diseases and reduce the risk of hospital admissions. There are incentive payments to GPs for each phase of the program.

Participants in the CVC program must be Gold Card holders with one or more chronic conditions that require complex care in an attempt to avoid hospitalisation. The eligibility criteria exclude those Gold Card holders who live in an aged care facility, have a condition that could be terminal within 12 months or already has a Commonwealth Department of Health and Ageing Aged Care Package.

Bupa Health Dialog is the organisation that has been working with DVA to identify the Gold Card holders and their main GP and to contact both regarding the program. GPs and DVA clients have commenced receiving these letters.

The Australian General Practice Network (AGPN) is offering FREE online education modules related to chronic disease management, in particular to practice staff involved in this project.

CVC Programs Social Assistance Service

DVA Gold Card holders enrolled in the CVC Program who are socially isolated may be referred to the CVC Programs social assistance service. The social assistance service is a short term intensive service (generally up to 12 weeks) to assist veterans and war widows to become engaged in community activities. The services are provided through the Veterans Home Care (VHC) Program, for assessment and service provision, but delivered differently from the VHC services already provided.

To refer a patient enrolled in the CVC Program for social assistance a specific referral form does not need to be used. A referral can be made by a GP using the practice letterhead, stating that the patient is enrolled in the CVC program, the date they were enrolled (date claimed) and that a Veterans Home Care assessment for social assistance under the CVC program is required. Referrals can be sent to VHC by fax to 8349 3510.

When enrolling patients into the CVC Program, the GP needs to explain what it means to be in the program, sharing their medical/health information and then obtain consent from the patient.
