Mental Health Shared Care Program

This program provides a free clinical service that focuses on people with low prevalence, severe and complex disorders.  This includes people who have been diagnosed with or are at risk of developing disorders such as Schizophrenia, Bipolar Disorder, Depression, Anxiety and Personality Disorders. The aim of the program is to assist GP’s to manage their patient’s mental health concerns and to help them function effectively in the community.

The benefits of using the service is that it is free to clients, it only requires a once off Mental Health Treatment Plan, and it provides clients with an unlimited amount of sessions, depending on their need.

The service provides a full mental health assessment of the client and the provision of ongoing treatment services to manage patients mental health functioning.  We employ a broad range of treatment modalities (e.g. CBT and Interpersonal Therapy) and have strong links with community and government services for onward referral and management.

Shared Care clinicians are experienced allied health professionals (Psychologists, Social Workers, Mental Health Nurses and Occupational Therapists) who are able to see clients either at GP rooms or on-site at GP partners Adelaide, 120 Hutt Street Adelaide.

Please contact the Division on 8112 1100 for further information or simply fax referrals to 8227 2220
